In 2000, Belgian demographer Michel Poulain and Italian physician Gianni Pes began mapping a statistical anomaly in Sardinia. In a cluster of villages in the Barbagia region, the concentration of male centenarians was unmatched anywhere else in the world. To mark the relevant municipalities on their map, they used a blue pen. The term Blue Zone was born.
A few years later, journalist Dan Buettner, working with National Geographic, extended the research to other regions of the world. Five zones were ultimately identified and validated through rigorous demographic analysis. These five regions constitute large-scale natural experiments in longevity — human populations that, without modern medical intervention, have solved part of the problem of biological aging in a reproducible, documented way.
The five Blue Zones
Sardinia — Barbagia, Italy. Sardinia has the highest documented concentration of male centenarians in the world. Unlike the global trend where women make up 80-90% of centenarians, Sardinia's male-to-female ratio is close to 1:1 — a demographic anomaly that has drawn geneticists' attention. Genomic studies have identified specific variants in genes governing oxidative-stress response and inflammatory regulation.
Okinawa — Japan. The Okinawa archipelago long held the world record for female longevity. The Okinawa Centenarian Study, running since 1975, has amassed one of the richest datasets on the biology of human longevity. Okinawan centenarians show remarkable biological markers: low chronic inflammation, reduced oxidative biomarkers and favourable lipid profiles. Notably, the generation born after World War II and exposed to dietary Americanisation shows significantly lower life expectancy than their elders.
Loma Linda — California, USA. Loma Linda is composed largely of Seventh-day Adventists. The Adventist Health Study, which followed more than 96,000 participants, documented that Adventists live on average 7 to 10 years longer than the Californian average. It is the only Blue Zone in an industrialised Western country.
Nicoya — Costa Rica. Studies of this population have revealed significantly longer telomeres among Nicoya centenarians. "Plan de vida" — a strong sense of purpose and reason for being — has been identified as one of the distinctive psychobiological factors.
Ikaria — Greece. The Greek island of Ikaria has a rate of people reaching 90 roughly three times the European average, with some of the continent's lowest rates of dementia and depression. Its inhabitants are described by researchers as people who "forget to die".
A predominantly plant-based diet, low in refined sugar
Across all five Blue Zones, the diet is largely made up of legumes, vegetables, whole grains, fruit and nuts. This diet shares several biologically relevant traits: high fibre density (microbiome substrate), richness in polyphenols (modulators of the sirtuin/mTOR pathways), and low glycaemic index (limiting IGF-1/insulin signalling). In Okinawa, the cultural principle "Hara hachi bu" — eating until 80% full — is a form of gentle caloric restriction practised since childhood, with documented effects on the mTOR and AMPK pathways.
Gentle, daily physical activity woven into life
What these populations share is moderate, regular physical activity woven into daily life — gardening, walking, manual work. This intensity corresponds to what Peter Attia calls "zone 2" — the intensity that optimises mitochondrial biogenesis, fat oxidation and insulin sensitivity. At the cellular level, this activity maintains high muscle NAD+ levels, activates the mitochondrial sirtuins SIRT1 and SIRT3, and stimulates autophagy.
Low chronic stress and ritualised decompression mechanisms
Blue Zone populations share ritualised decompression mechanisms: the afternoon nap in Sardinia and Ikaria, daily prayer among Adventists, the "moai" (lifelong social support group) in Okinawa, sabbath rest. Studies have shown that high levels of social cohesion correlate with longer telomeres and lower IL-6 levels.

A strong sense of purpose and community belonging
In Okinawa, "ikigai" (reason for being) is a central cultural concept. In Nicoya, "plan de vida" describes that sense of purpose structuring daily life. High levels of meaning and belonging are associated with lower baseline cortisol, better sleep quality, and reduced inflammatory markers.
The genetics of the Blue Zones: how much do they contribute?
Genetics is estimated to account for roughly 20-30% of extreme longevity in most twin studies, leaving 70-80% to environmental and behavioural factors. Okinawans who emigrated to the United States and adopted a Western lifestyle show life expectancy close to that of the local population — not their country of origin. This proportion means that most determinants of biological longevity are modifiable.
What the Blue Zones don't teach
An analysis by Saul Newman published in 2023 raised doubts about the reliability of civil registries in certain regions. This work underscores the importance of rigour in interpreting population-longevity data. The Blue Zones are also integrated biological and social systems. It is the coherence and persistence of the full set of factors across an entire lifetime that generates the documented biological trajectories — not any single isolated factor.
